Disability Support Worker - Brisbane South

Harmony Support Australia is seeking experienced, compassionate, and resilient Support Workers to join our team to provide person-centred supports in Brisbane Southside area.

This is an excellent prospect for professionals who are passionate about delivering high-quality, trauma-informed support while promoting independence, dignity, and community participation.

What you'll be doing

As a Support Worker, you will:

- Deliver person-centred support in accordance with the participant's NDIS goals and preferences.
- Implement Positive Behaviour Support strategies built in Behaviour Support Plans (BSP/PBSP).
- Build a trusting and therapeutic relationship with the participant.
- Support community access, appointments, daily living skills, and social participation.
- Maintain accurate case notes and incident reporting.
- Work collaboratively with families, behaviour support practitioners, and the broader multidisciplinary team.
- Promote a safe, respectful, and positive environment at all times.

- Minimum two years experience as a Support Worker working with challenging behaviours, Autism, implemented PBSB strategies
- Strong communication and interpersonal skills
- Ability to work independently and within a team
- Good knowledge of Diabetes management or willingness to learn
- Experience providing psychosocial support in a community or residential setting
- Ability to manage complex mental health situations
- High intensity supports, Personal care, Hoist /Sera steady experience
- Current NDIS Worker Screening Check
- Blue Card
- First Aid & CPR
- Driver’s Licence
