Mercy Health in the Newcastle region is seeking a Care Partner to deliver person-centred care management for older adults living at home. This full-time role requires coordination with participants, supporters, and interdisciplinary teams to ensure tailored and responsive services.

You will contribute to wellness, reablement and culturally safe care, supporting multiple care pathways while maintaining solid relationships and compassionate service delivery.
