Choose when and where you work while using your social work experience to improve clients' mental health, wellbeing and quality of life.About Care GuidanceCare Guidance exists to empower all families to find the right care for their loved ones.Our Wellbeing Program provides social-work-led support to older Australians across Support at Home, residential aged care and hospital settings.The program is growing rapidly, and we are looking to expand our current team of more than 30 Social Workers.Our ambition is to make social-work-led wellbeing support a core part of aged care across Australia, helping address the mental health, emotional and psychosocial needs that can often be overlooked as people age or enter care.About the roleWe are looking for experienced Social Workers to provide one-to-one support to older Australians receiving Support at Home services.
Care Guidance partners with Support at Home providers to deliver social work support to their clients as needs arise.You will receive referrals based on your availability and the areas you are able to service, visiting clients in their homes to provide either one-off or ongoing support depending on their individual circumstances and needs.Key areas of support may include:Social isolation and lonelinessGrief and loss counsellingAdjustment to ageing, illness and aged carePsychosocial assessments and supportMental health and emotional wellbeingFamily and carer supportAdvocacy and assistance navigating servicesAdministrative and practical supportSupporting clients experiencing complex psychosocial challengesThe role is highly flexible and referral-based.
You can nominate the days you are available and the areas you are comfortable servicing, with a minimum availability of one day per week.
Referrals are then offered based on client need, location and your availability.There are also opportunities to undertake regular work within Care Guidance's partner residential aged care homes, attending the same home on a weekly or fortnightly basis, depending on your interests and availability.
