Ungooroo Aboriginal Corporation – Singleton NSWLocated in the heart of the Hunter Valley, Ungooroo Medical Service is an Aboriginal Community-Controlled Health Service, which aims to deliver culturally appropriate primary health care services, advocacy and social and emotional support to Aboriginal and Torres Strait Islander people and their families in our community.Ungooroo Aboriginal Corporation is looking for an experienced ITC Care Coordinator - Full-Time (or Part time Available) to join our Integrated Team care program.

but we strongly encourage Aboriginal and Torres Strait Islander Peoples to apply.About the roleContribute to better treatment and management of complex chronic conditions for Aboriginal and Torres Strait Islander peopleImprove access to appropriate health care through care coordination and provision of supplementary servicesImprove access for Aboriginal and Torres Strait Islander people to culturally appropriate mainstream primary care.Provide culturally appropriate care.Maintain patient records in accordance with My Health Record systemData collection and reportingWork in collaboration with stakeholdersUnderstanding of WHS requirements and complianceAbout YouDemonstrate a patient-focused approach to careGenuine empathy and interest in patient needs.Excellent interpersonal and communication skills across all ages and social groups.Maintain absolute confidentiality around patient and practice informationDemonstrate knowledge and compliance with all relevant legislation and obligations affecting nursing and patient care.Maintain a vigilant attitude to accuracyAbility to work cooperatively and independentlyAbility to prioritise and organise your timeAlways represent Ungooroo in a confident and positive mannerQualifications and ExperienceQualification in health such as Registered Nurses, Enrolled Nurses or Aboriginal Health Workers.
Aboriginal Health Workers will have completed the Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice (consideration may be given to other appropriate qualifications in specific circumstances)Demonstrated understanding of the local health system and referral pathways.Please note: Applicants will be subject to a Working with Children Check and a Criminal History CheckFind out more about Ungooroo at or email ******.
