Job Description
Uniting Limited is hiring a Carer Wellbeing & Support Planner for the Carer Gateway to work with unpaid carers, assessing circumstances, priorities and impact using the Carers Star tool to create practical, goal-directed support plans.

You will provide information, guidance and individual support, arrange brokered services within funding limits and engage carers through coaching, peer support and community events.
