Expressions of Interest (EOI)
from dedicated clinicians to join our network of
Clinical Aged Care Assessors
for both in-home and residential assessments. With positions available
nationally across Australia

Daily travel is a core requirement of this role. You will be required to travel throughout your service area every day to meet business and client needs, providing support where it's needed most

Your Role:

High-Quality Assessments
: Conduct thorough Aged Care Assessments to identify the needs of older adults.
n
Service Referrals
: Facilitate referrals to essential services that support independent living.
n
Promote Functionality
: Actively promote the retention and maximization of function and capability in daily living.
n
Engage Stakeholders
: Work closely with families and carers, recognizing them as key stakeholders.
n
Compliance
: Ensure all assessments are conducted in line with Department of Health contractual requirements.

What You Bring:
n
n
At least 1 year working with older Australians, including client assessments and/or service delivery.
n
Tertiary education in a health-related discipline together with skilled registration with relevant association(e.g. AHPRA)
n
Proficiency in Word, Excel, Outlook, and client management systems.
n
Current Australian driver's licence, comprehensively insured vehicle, and the ability to travel within your service area.
n
