The Salvation Army is seeking a dedicated Case Manager to join our Supported Accommodation team in Caboolture.
You will provide crisis intervention, assessments, and case management to people experiencing or at risk of homelessness, working in a trauma-informed, client-centred framework.
You'll collaborate with internal teams and external stakeholders, delivering practical support, developing individualised plans, and assisting access to housing and services to improve long-term outcomes.
