The Opportunity
We are seeking an experienced Change Manager to lead practical change management activities for the HELF Everyday Choices transformation within St Vincent's Care Services.
This transformation requires strong engagement across aged care operations, facility leadership, finance, compliance, Digital & Technology (D&T;), external vendors, and frontline teams.
You will play a pivotal role in driving operational readiness, resident/family communications, billing and documentation updates, training execution, and long-term change adoption.
This maximum term full‐time opportunity is based in our Brisbane Group Head Office and onsite at our Residential Aged Care homes.
Key Responsibilities
Develop and execute fit‐for‐purpose change strategies and actionable plans (communications, sponsorship, coaching, training, and resistance management) aligned with project delivery timelines
Conduct thorough change impact assessments, readiness evaluations, and stakeholder analyses covering operational, compliance, billing, and documentation impacts
Lead targeted engagement across Care Services to build awareness and support
Coach change champions, facility managers, and business leaders to support frontline teams with varying levels of digital confidence
Support the design and delivery of clear, practical communications and user guidance (e.g., quick reference guides, leader briefings) tailored for frontline care and business support functions
Identify adoption risks early, implement mitigation strategies, and track adoption success measures (readiness,
training completion, engagement metrics) for project governance and executive stakeholders
About You
Demonstrated ability to tailor engagement, coaching, and enablement materials for frontline care teams operating in high‐demand environments
Skilled in identifying adoption barriers early, navigating complex stakeholder environments, and facilitating consensus
Ability to rapidly build working knowledge of aged care workflows, digital platforms, and billing processes, while utilising project tools to track change metrics
Proven change management experience leading and embedding transformational change in large, complex operational environments
High‐level experience in aged care, health, or human services is highly desirable, particularly supporting compliance, process, technology, or frontline workforce adoption
Bachelor's degree in Business Administration, Organisational Development, Healthcare Management, or a related field
Strong familiarity with change management frameworks (e.g., Prosci, Kotter) and project management principles
