Choose when and where you work while using your social work experience to improve clients’ mental health, wellbeing and quality of life.

About Care Guidance

Care Guidance exists to empower all families to find the right care for their loved ones .

Our Wellbeing Program provides social-work-led support to older Australians across Support at Home, residential aged care and hospital settings.

The program is growing rapidly, and we are looking to expand our current team of more than 30 Social Workers.

Our ambition is to make social-work-led wellbeing support a core part of aged care across Australia, helping address the mental health, emotional and psychosocial needs that can often be overlooked as people age or enter care.

About the role

We are looking for experienced Social Workers to provide one-to-one support to older Australians receiving Support at Home services. Care Guidance partners with Support at Home providers to deliver social work support to their clients as needs arise.





You will receive referrals based on your availability and the areas you are able to service, visiting clients in their homes to provide either one-off or ongoing support depending on their individual circumstances and needs.

Key areas of support may include:

Social isolation and loneliness

Grief and loss counselling

Adjustment to ageing, illness and aged care

Psychosocial assessments and support

Mental health and emotional wellbeing

Family and carer support

Advocacy and assistance navigating services

Administrative and practical support

Supporting clients experiencing complex psychosocial challenges

The role is highly flexible and referral-based. You can nominate the days you are available and the areas you are comfortable servicing, with a minimum availability of one day per week. Referrals are then offered based on client need, location and your availability.

There are also opportunities to undertake regular work within Care Guidance’s partner residential aged care homes, attending the same home on a weekly or fortnightly basis, depending on your interests and availability.

$58p/h + superannuation

Travel between clients paid at the applicable kilometre allowance

Flexible casual work with a minimum commitment of one day per week

Skills and experience

Bachelor of Social Work

Eligibility for membership with the Australian Association of Social Workers (AASW)

Experiencing dealing with trauma

Social work experience within health, mental health, palliative care and/or aged care

Previous experience providing one-to-one counselling or psychosocial support

Sound clinical knowledge and the ability to manage complex psychosocial issues

Strong interpersonal skills and the ability to build rapport and trust with clients, families and other stakeholders

Ability to work autonomously while effectively collaborating with health and aged care professionals

A genuine interest in supporting older Australians to live with dignity and improve their mental health and wellbeing

Current driver’s licence and access to your own vehicle

Current Police Check, NDIS Worker Screening Check and Working with Children Check, or willingness to obtain them
