Chronic & Complex Community Registered Nurse

This position is required to participate in afternoon, weekend and public holiday shifts.

The rotation to these shifts is minimal at this current time, and is subject to change at anytime.

It is a community health centre based position requiring travel across sites and into private residences.

This position provides care in the community to client's requiring, wound management, continence and catheter management, and supportive and palliative care, including end of life.

What you'll be doing

As a Chronic & Complex Community Registered Nurse, you will provide comprehensive nursing assessment, care coordination and evidence-based clinical interventions to support clients with diverse and often complex healthcare needs.

You will work autonomously while collaborating closely with multidisciplinary teams to deliver safe, effective and client-centred care.

- Conduct holistic nursing assessments and develop tailored care plans




- Coordinate and deliver evidence-based clinical care, monitoring and education
- Partner with clients, carers and families to achieve individual healthcare goals
- Manage complex caseloads while responding proactively to changing clinical needs
- Maintain high-quality documentation and contribute to clinical review and quality improvement activities

Selection Criteria

1. Relevant clinical experience in speciality.

2. Demonstrated ability to organise and prioritise work.

3. Demonstrated effective communication skills both written and verbal. Effective interpersonal skills showing initiative and leadership.

4. Demonstrated commitment to professional development, evidence-based practice, and an awareness of changes within the health care environment.

5. Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.

6. Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.

7. Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training

8. Current Drivers Licence valid within Australia and willingness to use for work purposes.
