About the Role

Working with the support of a Clinical Operations Manager, as Care Partner you will provide person-centred assessment, care and service options for receiving At Home Care Packages.

You will monitor budgets and maintain positive outcomes for the participant and the Vasey RSL Care brand.

You will have your own participant allocation, however, will work collaboratively within a growing team of friendly and capable Care Partners and Service Coordinators.

Travel around metropolitan Melbourne, including regional areas will be required as needed.

About You

To be successful in this role, you will be person-focussed and passionate about achieving high quality outcomes.

You will also have:

- Current AHPRA registration: Div 1 (RN); Div 2 (EN); or other allied health professional registration e.g. OT

- Demonstrated experience assessing, planning, implementing,



and evaluating quality care and the ability to set, review and monitor measurable and achievable goals within the care plan in a timely manner

- Ability to deliver services with a strong rights-based, person-led approach

- Strong cultural awareness and understanding of care needs relating to different cultural backgrounds

- Demonstrated organisational, time management and multi-tasking skills

- Strong computer skills: MS Word, Excel, PowerPoint, Outlook and use of client management systems

- Current driver's licence and ability to drive

- Willingness to undergo a National Police Check and have proof of the right to work in Australia
