The Role

As a Community Occupational Therapist, you'll provide high-quality, person-centred services to government-funded aged care clients living within the community.

This role will see you travelling throughout Perth's southern metropolitan region, including areas as far south as Port Kennedy, with all business-related mileage fully reimbursed.
Working closely with clients, families, caregivers and service providers, you'll deliver evidence-based interventions that promote independence, wellbeing and participation in daily life.
Key responsibilities include:





- Conducting comprehensive functional and home-based assessments
- Developing tailored therapy and rehabilitation programs
- Prescribing assistive technology, equipment and home modifications
- Supporting clients to maintain independence and safety at home
- Reviewing and adjusting care plans in line with client goals
- Providing education and support to clients, families and carers
- Collaborating with internal and external stakeholders to achieve positive outcomes
- Maintaining accurate clinical documentation and reports

About You

We're seeking a confident and self-sufficient Occupational Therapist who enjoys building strong client relationships and delivering exceptional customer service.
To be successful, you will have:

- Bachelor of Occupational Therapy or equivalent qualification
- Current AHPRA registration
- Minimum 2 years' experience in community, aged care, rehabilitation or similar settings
- Experience conducting functional assessments
- Experience prescribing assistive technology and equipment
- Experience recommending home modifications
- Strong clinical reasoning and problem-solving skills
- Excellent communication and stakeholder engagement abilities
- Knowledge of wellness, reablement and restorative care principles
- Current driver's licence and reliable vehicle
