Conduct recovery-focused assessments and contribute to the development of personalised support and recovery plans.
Regularly review client goals and progress, ensuring support remains responsive to individual needs and aspirations.
Work collaboratively with clinical and non-clinical services to provide coordinated, person-centred support.
Advocate for clients and help them navigate service systems,

reducing barriers to accessing the supports they need.
Develop and maintain positive partnerships with local service providers, the NDIA, and other key stakeholders.
Maintain accurate client records, case notes, and reports in line with organisational and program requirements.
Contribute to service quality, continuous improvement initiatives, and knowledge sharing across the team.
