ACCORD CARE
is a trusted provider of person-centered disability support services to NDIS participants. We deliver Supported Independent Living (SIL), in-home care, community participation, short-term accommodation, and community nursing.

About the Role
n We are seeking dedicated Support Workers to join our Supported Independent Living team in
Norton Sands
. In this role, you will support participants to live as independently as possible while engaging actively in their community.
n
Key Responsibilities
n
n
Assist participants with personal care and daily living activities, including meal preparation, household tasks, and medication reminders
n
Support participants to access the community, attend appointments, and engage in social and recreational activities
n
Follow individualised Behaviour Support Plans and Risk Assessments to ensure safe, high-quality care
n
Maintain accurate documentation, including shift notes, incident reports, and data collection
n
Collaborate with team leaders to implement and review participant support plans
n
Maintain skilled relationships in line with NDIS and organisational standards
n




Contribute to positive participant outcomes and team objectives
n
Be reliable, accountable, and person-centered in all aspects of care
n

Qualifications & Experience
n
n
Certificate III in Individual Support, Aged Care, or higher (or willingness to obtain)
n
Experience supporting participants with
complex diabetes
n
Competent in
BGL monitoring and recording
n
Ability to
administer insulin
(pen or syringe) safely
n
Experience testing and
recording ketone levels
n
Ability to follow diabetes care plans and protocols
n
Experience in mental health and challenging behaviours
n
Nursing experience highly regarded
n
