The Care Coordinator has clinical oversight of allocated residents within the allocated area of the facility and provides clinical leadership to the team of Registered Nurses (RNs), Enrolled Nurses (ENs) and Carers.
KEY RESPONSIBILITIES:
Supervise the delivery of care on a daily basis by the care team
Oversee assessments and documentation for residents, ensuring accurate care planning and relevant supporting documentation
Provide coaching, education and support to the team, addressing gaps in performance as required
Daily review of Riskman, investigating incidents as appropriate, recommending and implementing corrective actions to close gaps
Drive and monitor ACFI assessments and process to maximise funding
Investigation and management of clinical issues and complaints in collaboration with Service Manager
Conduct clinical audits and customer surveys, and lead actions to close identified clinical gaps
Review Resident of the Day (ROD) and ensure follow up on any identified dissatisfaction or gap is followed up appropriately
Actively involved in analysing KPIs monthly and addressing identified trends
Ensure the rostered direct care staff are appropriately allocated to provide the highest levels of care for the shift
Conduct performance appraisals for clinical staff within responsible area and delegate and coordinate the performance appraisal process for care staff
Ensure care consumables are managed responsibly and within budget
Encourage members of the team to actively demonstrate and promote the organisations philosophy of care and 3Rs culture in all activities

Requirements
SKILLS, KNOWLEDGE, EXPERIENCE & COMPETENCIES REQUIRED:
Skills, Knowledge and Experience:
National Police Clearance Certificate (no older than 6 months)
Registered Nurse (Registered with AHRPA) minimum five years
Previous Aged Care experience
High level of clinical expertise and currency of practice
Proven organisational and time management skills
Excellent interpersonal communication skills, both verbal and written.
Demonstrated ability to manage competing priorities successfully in a high stress environment
High attention to detail
Ability to work independently and as a team member
Ability to influence teams to achieve robust customer focus and clinical excellence
