Goulburn Valley Health is seeking a Social Worker for Sub Acute, Acute and Outpatients.
The Fixed Term Part Time role (64 hours per fortnight) supports patients in Palliative Care, Rehabilitation and Geriatric Evaluation and Management within a multidisciplinary team.
You will assess and intervene for diverse psychosocial needs, manage a clinical caseload, and collaborate with clinicians to support patients and families through changes in health and wellbeing.
