The Katherine West Health Board seeks a Punyu Yirdipirrki Care Coordinator (PYCC), an Aboriginal Health Practitioner or Registered Nurse, to lead CKD care coordination with cultural safety and autonomy.
You will collaborate with the Chronic Disease and Aboriginal Health Coaches, use the TKC data platform, and support region-wide integrated renal care across Katherine West with a patient-centred approach.
