Key responsibilities
The role includes medication reconciliation, clinical review, drug information provision and independent therapeutic decision-making to optimise patient outcomes.
Working as part of a multidisciplinary team, the position also contributes to service planning, quality improvement and regional outreach services.
This role suits pharmacists with established hospital experience seeking greater clinical responsibility and autonomy.
The role includes inpatient virtual clinical pharmacy and pharmacist-led outpatient clinics in pre-admissions, day infusions, renal and cardiology.
