Wellways Australia is seeking a Service Integration Coordinator to help establish a statewide connected care pathway across Tasmania. You'll map services, build referral networks and engage diverse communities to connect people with the right support at the right time.
You'll work with a multidisciplinary team, travel across Tasmania and play a key role in shaping a no-wrong-door approach to mental health services for adults 18+.
