The Royal Melbourne Hospital is seeking a Clinical Nurse Consultant to join the Residential In Reach (RIR) team, supporting older adults as they transition from hospital back to residential aged care.
You will work across hospital and residential aged care settings to provide proactive discharge planning, intensive post-discharge clinical support and coordinated care for up to four weeks, collaborating with the Geriatric Medicine team, residents and families to prevent readmissions.
