Care Connect, a leading not-for-profit home care provider, is seeking an experienced Duty Care Manager for a full time maximum-term contract through to October ****. The role supports clients, families and care teams across Victoria, NSW and Queensland, with hybrid work from Bella Vista.
You will coordinate client handovers, manage escalations and ensure safe, person-centred services when a Care Manager is unavailable. Strong collaboration and documentation are essential.
