About the Role
You will work within a
Supported Independent Living (SIL) environment
, providing person-centred support to participants with Huntington's disease who may have complex and progressively changing support needs.
Depending on the participant and your training and competency, your responsibilities may include:
Assistance with personal care, showering, dressing and continence support;
Supporting mobility, transfers, positioning and safe manual handling;
Mealtime assistance and following individual swallowing and nutrition plans;
PEG-related support where required and within your training and competency;
Medication assistance in accordance with medication plans and organisational procedures;
Supporting communication where speech or cognitive changes make communication more difficult;
Recognising and reporting changes in health, mobility, swallowing,
behaviour, cognition or general wellbeing;
Following clinical care plans, risk assessments and allied-health recommendations;
Supporting appointments, community access and meaningful activities;
Maintaining accurate progress notes, incident reports and other required documentation;
Working collaboratively with nurses, allied health professionals, families, Support Coordinators and the broader care team;
Providing emotional reassurance and maintaining a calm environment; and
Supporting participants to maintain as much
choice, independence, dignity and quality of life
as possible.
Who We Are Looking For
We particularly welcome applicants with previous experience supporting people with:
Huntington's disease, acquired brain injury, neurological conditions, complex physical disability, PEG feeding, dysphagia, complex personal care, palliative approaches or high-intensity daily personal activities.
Previous Huntington's disease experience is highly desirable; however, applicants with strong complex-care experience and a genuine willingness to undertake participant-specific training are also encouraged to apply.
You will need to be someone who is:
Patient, calm and emotionally mature;
Respectful and compassionate;
Comfortable working with progressive and complex disability;
Able to follow clinical instructions precisely;
Observant and able to recognise changes in a participant's presentation;
Reliable and punctual;
Confident with documentation and incident reporting;
Able to work collaboratively as part of a multidisciplinary team;
Comfortable providing personal care with dignity and sensitivity; and
Committed to maintaining qualified boundaries and participant confidentiality.
Qualifications & Requirements
Applicants should have, or be willing to obtain as applicable:
Certificate III/IV in Individual Support, Disability, Community Services or equivalent relevant experience;
NDIS Worker Screening Check;
ACT Working With Vulnerable People registration;
Current First Aid and CPR;
Current Australian driver licence;
NDIS Worker Orientation Module;
Relevant medication and manual-handling competencies; and
Additional participant-specific clinical training where required.
Training and Clinical Support
Because Huntington's disease is progressive and each participant presents differently, successful candidates will be expected to undertake
participant-specific training and competency assessment
before independently performing complex or high-intensity support tasks.
Haven Care Australia works collaboratively with clinical and allied-health professionals to ensure staff understand the participant's current care requirements and are appropriately prepared as those needs change.
