Role Description
The Support Coordination Level 2 role is a contract position based in Geelong, VIC, with a hybrid work arrangement that allows a mix of in-office and work-from-home days. The role involves working with neurodivergent individuals and their families to understand their goals, assess support needs, and coordinate NDIS and community-based services. Day-to-day tasks include developing and reviewing support plans, connecting participants with appropriate service providers, monitoring service delivery, and addressing issues or barriers to effective support. The Support Coordinator will regularly communicate with participants, families, and providers, maintain accurate records, and ensure that supports are aligned with participant preferences, rights, and funding guidelines.
The role also includes supporting self-advocacy for participants, assisting with troubleshooting service-related issues, and providing clear information that helps families make confident decisions.
Qualifications
Candidates should possess robust Analytical Skills to interpret plans, funding, reports, and participant needs, and to identify gaps or improvements in supports.
Candidates should possess Technical Support and Troubleshooting skills to navigate NDIS portals, digital records, and service systems, and to resolve basic technical or process-related issues for participants.
Candidates should possess Customer Support and Communication skills to engage respectfully with neurodivergent individuals and families, build trust, manage expectations, and provide clear, empathetic guidance.
Relevant experience in support coordination, case management, disability services, or community services, ideally with neurodivergent people and NDIS participants.
Knowledge of the NDIS framework, funding categories, and local Geelong and wider VIC support networks.
Ability to work autonomously in a hybrid environment, manage competing priorities, and maintain accurate documentation, time recording, and reporting.
Commitment to neurodiversity-affirming practice, cultural sensitivity, and non-discriminatory, person-centered support.
A relevant qualification in social work, psychology, disability studies, community services, or a related field (or equivalent experience).
