Newcastle and Hunter Community Health is seeking compassionate, reliable and experienced Support Workers to join our community care team.
As a Support Worker, you will provide personalised assistance that helps clients maintain their independence, wellbeing and connection with their community.

About the Role
Your responsibilities may include:
Providing person-centred support to clients in their homes and community
Assisting with daily living activities
Supporting personal care needs where required
Assisting clients with community access and participation
Providing transport and assistance with appointments or activities
Supporting clients to work towards their individual goals
Following individual care and support plans
Maintaining accurate service records and documentation
Communicating professionally with clients, families and team members
Promoting client dignity, choice, independence and wellbeing
Following workplace health, safety and quality procedures
About You
We are looking for someone who is caring, dependable and comfortable working independently in the community.
You will ideally have:




Certificate III or IV in Individual Support, Aged Care, Disability, Community Services or a relevant equivalent qualification
Relevant experience in a support or community care role
Strong communication and interpersonal skills
A respectful and person-centred approach
Valuable time-management and organisational skills
Ability to work both independently and collaboratively
Flexibility to work across a rotating roster where required
Essential Requirements
Successful applicants may be required to provide:
Australian unrestricted driver's licence
Reliable, registered and appropriately insured vehicle
Current National Police Check or willingness to obtain one
Current First Aid and CPR certification
Relevant Working with Children or Working with Vulnerable People clearance where applicable
Smartphone and internet access
Relevant skilled referees
Evidence of required qualifications and training
Additional requirements may apply depending on the services and clients supported.
