Centacare Catholic Family Services is seeking a Case Manager to support families with children from unborn to 18 years, delivering trauma-informed, family-centred interventions. You will coordinate assessments, plan cases, and provide practical in-home support to strengthen parenting capacity and child safety.
You will lead a multidisciplinary response, using NCFAST and other tools, and collaborate with DHS and DCP to address risk factors while promoting culture and community connections.
