Key Responsibilities:
- Provide High-Quality Nursing Care: Deliver compassionate,
patient-centred care in a general practice setting, ensuring patients feel valued and supported.
- Clinical Support: Assist GPs with tasks such as immunisations, wound care, chronic disease management, and health assessments.
- Care Coordination: Contribute to the development and implementation of care plans to support patient health needs.
- Health Education: Educate patients on preventive care, treatment plans, and overall well-being.
- Patient Records: Maintain accurate and up-to-date electronic health records, ensuring seamless coordination of care.
About You:
- Registered Nurse: You hold a current registration with AHPRA as a Registered Nurse.
- Experienced in General Practice: You bring experience in a primary care setting and have a strong understanding of chronic disease management and clinical assessments.
- Great Communicator: You have excellent communication skills and enjoy building rapport with patients and team members alike.
