We are currently seeking a compassionate and experienced Registered Nurse - Immuniser to join our Aged Care at Home team on a permanent part-time basis, servicing clients across Currimundi and surrounding Sunshine Coast suburbs.This is an prospect to make a meaningful difference while working autonomously in the community, delivering evidence-based nursing care and supporting clients to live safely and comfortably in the place they call home.About the RoleAs a Registered Nurse, you will provide holistic, client-centred nursing care to older people living in the community. Working collaboratively with clients, families, carers and multidisciplinary teams, you will assess, coordinate and deliver clinical services that promote wellbeing, independence and quality of life.Key responsibilities include:Conducting comprehensive nursing assessments and developing individualised care plansCoordinating and managing client care in partnership with clients, families and healthcare professionalsDelivering evidence-based clinical care, including wound management, medication administration and chronic disease managementMonitoring, evaluating and documenting client outcomes in accordance with professional and legislative standardsIdentifying changes in client conditions and implementing appropriate interventions or referralsSupporting preventative health,
health promotion and early intervention strategiesMaintaining accurate and timely clinical documentationContributing to quality improvement initiatives and clinical best practiceAbout YouYou are a caring and capable Registered Nurse who enjoys working independently while remaining connected to a supportive team. You have a genuine commitment to delivering high-quality care and improving outcomes for older people in the community.To be successful, you will have:Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)Demonstrated competence in contemporary nursing practiceExperience in community nursing, aged care, hospital or primary healthcare settings (highly regarded)Strong assessment, clinical reasoning and care planning skillsExcellent communication, interpersonal and stakeholder engagement skillsThe ability to work autonomously and collaboratively within a multidisciplinary teamSound computer and mobile technology skillsA current Queensland Driver Licence and willingness to travel between client homesDesirable Skills & ExperienceExperience in care coordination and case managementQualifications or experience in immunisation services and vaccine administrationWound management and chronic disease management experienceKnowledge of the Aged Care Quality Standards and community care sectorWhat We OfferAs part of the Ozcare family, you'll enjoy a supportive and values-driven workplace along with a range of employee benefits,
