ABOUT THE ROLE

The Bays Healthcare Group is seeking a proactive, compassionate and organised Enrolled Nurse - Discharge Planner to support the safe, timely and person-centred discharge of patients from hospital to their home or other appropriate care settings.

Working collaboratively with patients, families, carers and the multidisciplinary team, the Discharge Planner will coordinate care transitions, identify and address barriers to discharge, and facilitate access to appropriate community and support services. This role is integral to ensuring continuity of care, enhancing the patient experience and supporting efficient hospital operations while maintaining a strong focus on patient safety, dignity and individual needs.

This is a 6-month maximum term role of 15 hours per fortnight.

KEY RESPONSIBILITIES

- Support the planning, coordination and facilitation of safe, timely and person-centred patient discharges.

- Identify patients with complex discharge needs and collaborate with the multidisciplinary team to develop, implement and review appropriate discharge plans.

- Liaise with patients, families, carers, internal departments and external service providers to coordinate ongoing care and support following discharge.

- Assist with referrals to community health, aged care, rehabilitation and other relevant services to promote continuity of care.

- Coordinate discharge requirements, including equipment, transport, follow-up appointments and community-based services.

- Monitor discharge planning progress, proactively identify potential barriers and escalate issues to the appropriate members of the healthcare team.

- Participate in multidisciplinary team meetings and contribute to collaborative, patient-centred discharge planning discussions.

- Provide clear and appropriate education and information to patients and their families regarding discharge arrangements, available services and post-discharge care requirements.





- Maintain accurate, timely and confidential clinical documentation in accordance with organisational policies, procedures and professional standards.

- Support effective communication and collaboration across clinical and non-clinical teams to facilitate efficient patient flow and positive discharge outcomes.

ABOUT YOU:

The successful candidate will demonstrate the following skills, experience and attributes:

- Relevant qualification in nursing and AHPRA registration.

- Previous experience in discharge planning, care coordination, case management or a similar role within an acute healthcare or hospital setting.

- Understanding of complex discharge planning, care transitions and the coordination of community-based health and support services.

- Knowledge of relevant health services, aged care pathways, rehabilitation services and referral processes.

- Strong organisational, time management and problem-solving skills, with the ability to manage competing priorities and meet discharge timeframes.

- Excellent communication and interpersonal skills, with the ability to build effective relationships with patients, families, carers and multidisciplinary teams.

- A compassionate, patient-centred approach that recognises individual needs, preferences, cultural considerations and the importance of continuity of care.

- Demonstrated ability to identify and escalate discharge barriers, coordinate resources and work collaboratively to achieve positive patient outcomes.





- Strong attention to detail and the ability to maintain accurate, confidential and timely clinical documentation.

- Commitment to patient safety, continuous improvement, professional standards and compliance with relevant legislation and organisational policies.
