Sunshine Coast Hospital and Health Service – Birtinya QLD is seeking a Senior Social Worker for the Geriatric Evaluation and Management and Hospital in the Home (GEM / HITH) team at Sunshine Coast University Hospital, Birtinya.
The role focuses on high‐level social work in the home visiting program and supports discharge planning for older patients.Remuneration pro rata up to $ p.a. (salary rates $–$ p.h.), with % superannuation and 17.5% annual leave loading.
