About the role:
Western Health is seeking an experienced and motivated mental health clinician to join our Community Mental Health Team as a Case Manager.
This is an opportunity to provide meaningful, recovery-oriented care to people experiencing complex mental health needs while working within a supportive multidisciplinary team. You will partner with consumers, families, carers and service providers to promote safety, wellbeing, recovery and participation in the community.
Key responsibilities:
As a Community Mental Health Clinician and Case Manager, you will manage an allocated caseload and provide comprehensive assessment, care planning, clinical intervention and service coordination.
You will:
- Develop, coordinate and regularly review individualised care plans.
- Build therapeutic relationships with consumers and support their recovery goals.
- Assess and monitor mental state, wellbeing and clinical risk.
- Work collaboratively with families, carers and the multidisciplinary treating team.
- Coordinate care with GPs, community agencies and other service providers.
- Apply supported decision-making and the principles of the Mental Health and Wellbeing Act 2022.
- Maintain timely and accurate clinical documentation.
- Contribute to the team’s duty function and provide continuity of care when required.
- Advocate for consumers and support clear communication across their care network.
About you:
You will be a reliable, organised and compassionate clinician who is comfortable working both independently and as part of a multidisciplinary team.
You will have:
- At least two years of experience providing clinical services to people with mental health needs, ideally within community mental health.
- Experience in assessment, care planning, intervention and clinical review.
- The ability to identify and respond appropriately to clinical risk.
- A strong understanding of recovery-oriented, person-centred and family-inclusive practice.
- Knowledge of the Mental Health and Wellbeing Act 2022.
- Strong written, verbal and interpersonal communication skills.
- The ability to manage competing priorities and maintain accurate documentation.
- Experience collaborating with primary health,
community and external service providers.
- A current Victorian driver’s licence and the ability to drive fleet vehicles.
Professional requirements:
Applicants must meet the qualification and registration requirements for one of the following disciplines:
- Registered Psychiatric Nurse Grade 3
- Occupational Therapist Grade 3
- Social Worker Grade 3
- Psychologist Grade 2
