The Opportunity:
We are seeking a Care Partner to join our growing team. This is a leadership-oriented role suited to someone with strong care management experience.
You will be responsible for delivering high-quality, person-centred care coordination to clients with varying and often complex needs.
Full-time or part-time arrangements are available.
Remuneration will be aligned with experience and clinical background.
Key Responsibilities:
Your responsibilities will include:
- Delivering care management in line with the current Aged Care Act and organisational policies, within allocated budgets and timeframes
- Developing, implementing, and reviewing comprehensive person-centred care plans
- Coordinating and liaising with internal teams, service providers, GPs, and allied health professionals
- Monitoring client outcomes and adjusting care strategies to ensure optimal service delivery
- Supporting clients and their families to make informed decisions about care
- Managing client enquiries, concerns, and complaints in a timely and skilled manner
- Maintaining accurate documentation and compliance with regulatory requirements
- Building and maintaining robust relationships with external stakeholders (e.g. ACAS, community services)
- Contributing to continuous improvement initiatives within the Support at Home program
- Promoting high standards of customer service and organisational values
About You:
We are looking for someone who brings strong care management expertise, with a genuine commitment to improving client outcomes.
You will have:
- Minimum 1 year experience managing a caseload of clients with complex needs, ideally within Home Care Package or Support at Home programs
- Strong assessment, care planning, implementation, and evaluation skills
- Excellent communication and stakeholder engagement abilities
- Sound problem-solving and critical thinking capability
- Ability to work autonomously while contributing to a collaborative team
Highly Desirable:
- Experience working within Consumer Directed Care and aged care compliance frameworks
- Clinical qualification as a Registered Nurse or Allied Health Practitioner
Working Arrangements
- Primarily office-based role
- Regular client visits within the community
- Flexible arrangements available for the right candidate
Essential Requirements
- Current Working With Children Check (or willingness to obtain)
- Current influenza vaccination
- Australian working rights
- National Police Check (or ability to obtain) or NDIS Screening checks
