Grace Ability is looking for a caring, reliable and experienced Disability Support Worker to join our team.

We are seeking someone who is passionate about supporting people with disability to live safely, independently and with dignity. You will provide person-centred support within a Supported Independent Living (SIL) environment.

Your Role

Duties may include:
Personal care and assistance with daily living
Mobility and transfer assistance
Medication assistance
Meal preparation
Domestic assistance and maintaining a safe home setting
Community access, appointments and social activities
Supporting participants to achieve their individual goals
Following participant support plans and risk assessments
Completing progress notes and required documentation
Reporting changes in participant health, wellbeing or support needs
Providing respectful, professional and person-centred support

Compassionate, patient and respectful
Reliable and punctual
Confident working independently
A strong communicator




Qualified and able to maintain appropriate boundaries
Experienced in disability support or a similar care role
Available for a variety of shifts

Requirements

Successful applicants should have:
Current NDIS Worker Screening Check
Current First Aid & CPR
Australian driver’s licence
Reliable vehicle where required
Right to work in Australia
Relevant disability/support qualifications or experience preferred

Day, afternoon, weekend and sleepover shifts may be available.
