Bayside Health in Caulfield Hospital seeks an experienced MATS nurse to triage referrals from community and within the Alfred Care Group. You will prioritise needs, allocate outreach workloads, and support patient flow through rapid assessments and transfers.

The role requires registration with AHPRA, 5+ years of post-graduate experience, and solid knowledge of complex health conditions. Postgraduate gerontology or palliative care qualifications are desirable.
