Catholic Healthcare Limited in Sydney is seeking an Aboriginal or Torres Strait Islander Identified Aged Care Assessor to join our ACAS team. This permanent full time role offers flexibility and local travel across metro and regional locations.

You will conduct assessments using the Integrated Assessment Tool to identify needs and refer to appropriate home support services, collaborating with clients, families and multidisciplinary teams to deliver person-centred care.
