The role involves travel to patients’ homes and residential facilities to provide high-quality clinical pharmacy care as part of a multidisciplinary team.

You will manage medication histories, reconciliation, therapy reviews and counselling, ensuring secure, effective and timely use of medicines in home and community settings.
