PM and Night shifts.What You'll Do:Carry out initial and ongoing assessment, planning, and management of nursing care for residentsDevelop personal care plans to ensure residents' individual needs are identified and met, and monitor any changes in care plansParticipate in quarterly care plan evaluations, or more frequently as neededEnsure all documentation required for quality care is prepared, maintained, monitored, evaluated, and up to date in accordance with SCC policies and proceduresEnsure completion and accuracy of all ACFI claims
