About the Role
Assist participants to access their community and attend appointments
Deliver clinical and personal care: manual handling, hoisting, continence, PEG feeding, medication administration
Support daily living: hygiene, meals, mobility, transport
Monitor health changes and follow clinical care plans
Maintain accurate case notes and incident reports
Collaborate with families, allied health and nursing teams
Ensure a safe, clean, and engaging home environment
About You
Minimum 3-6 months' experience or Min Certificate III/IV in Disability, Individual Support, Nursing, or related fields.
Experience with high physical needs and clinical care
Confident in manual handling, medication administration, and hoist transfers
NDIS Worker Screening, First Aid, valid Driver's Licence, and insured vehicle
