About the Role:
We are seeking an Occupational Therapist to join a dynamic aged care allied health provider in South East Melbourne, VIC. This is a part time role of 2 days per week, supporting older people to remain safe and independent in their own homes. You will provide person-centred assessment and recommendations, including home modifications such as ramps and rails, and work closely with a supportive Team Leader.
- Conduct assessments of older people in their own homes and recommend appropriate supports
- Assess for and prescribe minor home modifications, including ramps and rails, and assistive equipment
- Develop and maintain care plans and clinical goals
- Work within legislative, regulatory and scope of practice requirements
- Communicate professionally with clients, families and other stakeholders and apply a person-centred approach
- Keep documentation and reports to a high standard, in line with company and medico-legal requirements
- Seek advice and clinical support from your Team Leader when needed
- Follow company policies and practices
- Attend team meetings as required

About You:
- Tertiary qualification in Occupational Therapy
- Current AHPRA registration
- Experience prescribing home modifications, including ramps and rails (essential)
- Experience working in community or aged care settings (desirable)
- Person-centred, collaborative approach with strong communication and documentation skills
- Ability to work autonomously and manage a community caseload
- Current driver's licence
- Eligible to work in Australia
