About the Role:

We are seeking a Physiotherapist to join a dynamic allied health provider in Geelong, VIC. You will deliver timely, evidence-based physiotherapy within a transition care and slow stream rehabilitation setting, helping older people regain independence and return home with confidence. You will work closely with a supportive Team Leader and a collaborative multidisciplinary team.

- Deliver high quality, timely, evidence-based physiotherapy within a rehabilitation setting
- Work within legislative, regulatory and scope of practice requirements
- Conduct clinical assessments and develop and maintain care plans and clinical goals
- Support clients to build strength, mobility and independence, and to transition safely home
- Communicate professionally with all stakeholders and apply a person-centred approach
- Keep documentation to a high standard, in line with company, site and medico-legal requirements
- Seek advice and clinical support from your Team Leader when needed
- Prepare reports as required
- Liaise with partner organisations in line with contractual expectations

- Attend team meetings as required

About You:

- Tertiary qualification in Physiotherapy
- Current AHPRA registration
- Experience in rehabilitation, transition care or working with older people (desirable)
- Demonstrated competence delivering therapy across a wide range of clinical conditions and service settings
- Experience meeting key performance measures
- Experience managing others to achieve key performance measures (desirable)
- Valid driver's licence
- Eligible to work in Australia
