People Who Care is a not-for-profit organisation with over 45 years of service supporting older people, people with disability, and individuals experiencing social crisis. We work in partnership with our community to deliver services that promote independence, dignity and wellbeing.

We are seeking an experienced and motivated Care Coordinator – Support at Home to join our team in this newly created role. Based in Guildford, this position plays a key role in coordinating high-quality, person-centred services under the Australian Government’s Support at Home program, enabling older people to live independently, safely and with dignity in their own homes.

Working as part of a supportive and committed team, you will coordinate services that are wellness-focused, responsive and aligned with the Aged Care Quality Standards and Support at Home requirements. This role is ideal for an organised and proactive professional with strong communication skills and experience in aged care, home care or community services.

This position is offered full time on a fixed-term 6 month contract, with immediate commencement. Hourly pay rate: $45.46

Key Responsibilities

- Coordinate and administer Support at Home services in accordance with program requirements and the Aged Care Quality Standards

- Undertake service planning, care coordination, budget management and administration of client funding

- Develop, implement and regularly review individualised service delivery plans in partnership with clients and their support networks

- Coordinate services with clients, support workers, allied health professionals, health professionals, families and other stakeholders

- Monitor clients who may be at risk and proactively review services to ensure their needs, goals and circumstances continue to be appropriately supported

- Maintain accurate and up-to-date client records, case notes,



service information and program data

- Monitor service delivery, expenditure and client budgets to ensure services are appropriate, effective and within available funding

- Identify and resolve operational and service delivery issues promptly and effectively

- Support wellness, reablement, independence and person-centred approaches to service delivery

- Ensure services are delivered in accordance with relevant legislation, policies, procedures and contractual requirements

- Ensure appropriate health, safety, risk management and quality requirements are maintained

- Build and maintain positive relationships with clients, families, service providers and community stakeholders

- Contribute to continuous improvement and the effective implementation of the Support at Home program

Essential Criteria

- Certificate IV or higher qualification in Aged Care, Disability, Individual Support, Community Services or a related discipline, and/or significant relevant experience in home care or community services at a coordination level

- Demonstrated experience in aged care, home care or community services, with an understanding of person-centred and wellness-focused service delivery

- Understanding of the Support at Home program, aged care funding and/or experience working within a similar home care setting

- Experience developing and reviewing individualised service plans and coordinating services to meet client needs and goals

- Experience building collaborative partnerships with clients, families,



service providers and other stakeholders

- Knowledge of relevant community resources, services and support networks

- Strong computer skills, including Microsoft Office

- Confidence using Client Record Management and/or care management systems

- Excellent written and verbal communication skills

- Strong organisational skills and the ability to manage competing priorities and deadlines

- Ability to work effectively in a fast-paced environment while maintaining a high level of accuracy and attention to detail

- Commitment to person-centred, culturally responsive and respectful service delivery

- Australian work rights

- Current Western Australian Driver’s Licence

- Current National Police Clearance (issued within the last 6 months)
