ABOUT THE ROLE:
We are looking for a compassionate and skilled Social Worker to join our team at Corryong Health. In this role, you will provide trauma-informed care tailored to individual needs, with a focus on promoting emotional, social, and psychological wellbeing for all people we serve.
You will work collaboratively with patients, families, carers, and the broader healthcare team to support holistic care planning and service delivery. A key part of this role involves supporting service system navigation, care coordination, and management to ensure smooth and seamless transitions between inpatient, ambulatory, and residential care settings.
As a valued member of our interdisciplinary healthcare environment, you will also contribute to peer learning in trauma-informed care that is culturally sensitive and respectful of the diverse communities and population groups we are proud to serve.

ABOUT YOU:
We welcome applications from people who bring the following:
- A Bachelor's or Master's degree in Social Work and eligibility for registration with the Australian Association of Social Workers (AASW)
- Demonstrated ability in providing clinical assessments that support outcome-focused care plans for people experiencing high rates of vulnerability and complex care needs, including:
- Intergenerational trauma
- Cognitive decline or capacity concerns
- Neurodiversity
- Family violence
- Sexual assault
- Substance use disorders
- Mental health concerns
- Housing insecurity or homelessness
- Financial stressors
- Excellent interpersonal and communication skills that foster trusting, respectful, therapeutic, and professional relationships — both within and outside the organisation
- A demonstrated ability to work within a multidisciplinary team through collaborative, inclusive,
and solution-focused approaches that support effective health outcomes for clients, carers, and families
- A genuine commitment to the Victorian Public Sector (VPSC) Code of Conduct, Equal Employment Opportunity, Work Health & Safety, and Cultural Diversity principles
The following are desirable but not essential — we encourage you to apply even if you are still developing in these areas:
- Post-graduate training and/or experience in Mental Health, Addiction Medicine, therapeutic interventions, or group therapy
- Experience navigating the Victorian Civil and Administrative Tribunal (VCAT) and advocating for clients, families, and carers
- A comprehensive understanding of the legislative landscape relating to the Multi-Agency Risk Assessment and Management (MARAM) Framework and Information Sharing Entities (ISEs), to ensure victim survivors of family violence — including children — are appropriately supported, and adults who use violence are held to account
- Solid knowledge of Advance Care Planning (ACP), Enduring Medical or Financial Power of Attorney (EMPOA/FPOA), Guardianship, and Voluntary Assisted Dying (VAD)
