We’re looking for an experienced Care Coordinator to join our disability support team.
This is a hands-on role working closely with our participants, families, support workers and management to make sure supports are running smoothly and participants are receiving the care they need.
Key responsibilities include:
- Coordinating participant and staff supports
- Calling participants and staff for regular check in, in person or over the phone
- Supporting and guiding support workers
- Organising participant activities (Dungeon & Dragons, Bowling and organise other activities)
- Following up incidents, concerns and changes in participant needs
- Liaising with families, Support Coordinators and allied health providers
- Managing new participant enquiries, intake and onboarding in person and online
- Building relationships with Support Coordinators and referral partners
- Helping identify and bring in recent participants
- Supporting after-hours rostering, staff matching and service delivery
- Maintaining participant documentation and records
We’re looking for someone who is organised, confident, people-focused and able to take initiative with previous experience in disability or community services.
Preferred qualifications:
- Bachelor of Business Administration
- BSB50420 Diploma of Leadership and Management
- BSB50120 Diploma of Business
- Or a relevant Disability / Community Services qualification
Previous NDIS or disability sector experience is highly regarded.
