As we continue to grow our Seniors Services team, we are seeking an experienced and compassionate Clinical Care Advisor - Registered Nurse to provide clinical leadership, care coordination and case management support to older people living in the community.

About the role
Reporting to the General Manager, the Clinical Care Advisor - Registered Nurse will manage a dedicated client caseload and work closely with Elders and Seniors, families, carers, Support Workers, Care Advisors, and internal and external health professionals to deliver high-quality, person-centered care.

This role combines nursing expertise with care coordination and clinical leadership to ensure clients receive the right support, at the right time, while maintaining their independence and wellbeing at home.

What you'll be doing

- Clinical Care and Assessments
- Undertake comprehensive clinical and care assessments.
- Develop, implement and review client care plans.
- Monitor health status and clinical risks.
- Support wellness, reablement and restorative care approaches.
- Coordinate referrals to allied health, medical and community services.

- Care Coordination and Case Management

- Coordinate services across Support at Home, CHSP, NATSIFAC and other funded programs.
- Manage client caseloads, service delivery and reviews.




- Monitor client budgets and funding utilisation.
- Work collaboratively with multidisciplinary teams to achieve positive outcomes.

- Clinical Service Delivery - Provide nursing services including:

- Medication management
- Continence care
- Catheter care
- Chronic disease support
- Health education and health promotion
- Clinical monitoring and follow-up care

- Leadership and Quality Improvement

- Provide clinical guidance and support to staff.
- Manage a team of Care Advisors
- Participate in clinical governance and quality improvement activities.
- Support compliance with Aged Care Quality Standards and aged care legislation.
- Contribute to audits, accreditation and service development initiatives.

Essential selection criteria

1. Current unrestricted registration as a Registered Nurse with AHPRA.
2. Experience in Aged Care, community nursing, care coordination or case management.
3. Strong clinical assessment and decision-making skills.
4. Knowledge of Aged Care Quality Standards and aged care programs.
5. Experience managing client caseloads and care planning.
6. Excellent communication and relationship-building skills.
7. Demonstrated ability to work independently and within a multidisciplinary team.
8. Commitment to culturally secure and person-centred practice.
9. Current Driver Licence (minimum Ps).

Highly regarded

1.



Experience working within Aboriginal and Torres Strait Islander health or community services.
2. Understanding of Support at Home and client budget management.
3. Experience supporting quality improvement and accreditation processes.
4. Leadership, mentoring or clinical supervision experience.

Additional Requirements

- National police check
- NDIS Worker Screening Check
- Current First Aid and CPR certification.
- Evidence of vaccinations as required by legislation and organisational policy.
