Community Care Partner

Banksia is seeking an organised, compassionate and client-focused Care Partner to join our Community Care team. This role supports older people to remain independent, safe and well at home by coordinating responsive, person-centred care and services.

Working closely with clients, families, care staff and health professionals, you will assist with client onboarding, assessments, care planning, budget monitoring and ongoing reviews. You will also maintain accurate client records, coordinate referrals and help ensure services are delivered in line with individual needs, organisational requirements and relevant care standards.

This is a part-time position offering:

- 7 days per fortnight (52.5 hours per fortnight)
- Flexible working hours between 8.00 am and 5.00 pm
- Commencement available during November

Banksia is a highly respected, single-site not-for-profit and charitable organisation delivering quality aged care services in the beautiful seaside village of Broulee, located on the South Coast of NSW. We provide care and support across three integrated services:

- 121 independent living villas in our retirement village
- An 80-bed residential aged care facility
- Community care services

What you’ll do:

- Support new client assessments, onboarding and documentation for Support at Home, CHSP and private clients.
- Develop and review individualised care plans and budgets with clients.
- Monitor client budgets, funding, quarterly statements, assistive technology and home modification funding.
- Maintain accurate and confidential client records, including care plans, progress notes, medications, incidents and assessments.
- Coordinate referrals and health information with GPs and allied health professionals.
- Support care plan reviews and work closely with the Senior Care Manager, roster team and care staff.
- Promote client wellness, reablement, independence and holistic care.
- Help monitor service quality,



staff feedback and compliance with organisational requirements.
- Purchase approved client consumables in consultation with management.
- Undertake other administrative duties within the scope of the role, including meeting minutes and accounts support.

What you'll need:

- Certificate IV in Aged Care, or an equivalent qualification
- At least two years’ experience working in the aged care sector
- Experience using client database and record management systems
- Proficiency in Microsoft Office applications
- Knowledge of the Aged Care Act 2024 and an understanding of the Support at Home guidelines
- Strong communication, interpersonal and problem-solving skills
- A friendly, positive and supportive approach to customer service, with the ability to assist clients from diverse backgrounds and circumstances
