About the Role

We are seeking compassionate, reliable, and experienced Disability Support Workers (male or female) to join our growing team in the Moreton Bay Region.

We are specifically looking for support workers with experience supporting participants with high and complex needs, including individuals who require intensive, specialised, and individualised support to participate safely and meaningfully in the community.

This role focuses exclusively on community access and community-based support, assisting participants to engage in social activities, attend appointments, participate in recreational programs, exercise, and develop their independence.

The successful candidates will be confident supporting participants with complex behaviours, communication difficulties, mental health challenges, and diverse support requirements. You will play an important role in promoting independence, dignity, emotional wellbeing, and meaningful community participation.

Key Responsibilities

- Provide person-centred support tailored to participants with high and complex needs.
- Facilitate community access, social activities, appointments, recreational programs, and outdoor activities.
- Support participants with complex behaviours using appropriate Positive Behaviour Support strategies.
- Assist participants with emotional regulation, mental health, communication, and general wellbeing.
- Support individuals with limited verbal communication or who are non-verbal, where required.
- Follow individual support plans, behaviour support plans,



risk assessments, and established safety procedures.
- Promote choice, control, dignity, independence, and social inclusion.
- Recognise and respond appropriately to changes in participants' needs, behaviours, and wellbeing.
- Maintain accurate shift notes, incident reports, and other required documentation.
- Communicate effectively with families, coordinators, allied health professionals, and the wider support team.
- Ensure participants are supported safely and respectfully during all community-based activities.

Essential Requirements

- Relevant qualifications in Disability, Mental Health, Community Services, or a similar field.
- Demonstrated experience supporting participants with high and complex needs.
- Experience supporting individuals with complex behaviours, mental health conditions, and/or significant communication support needs.
- Experience implementing Positive Behaviour Support strategies and following individualised support plans.
- A solid understanding of person-centred, trauma-informed, and strengths-based support.
- Current NDIS Worker Screening Check.
- Current Working With Children Check (Blue Card).
- Current First Aid and CPR certification.
- Current driver's licence and willingness to support participants in the community.




- Strong communication, documentation, and problem-solving skills.
- Reliable, patient, empathetic, and professional, with the ability to remain calm in challenging situations.
- A genuine passion for empowering people with disabilities to achieve their goals and participate in their community.
