About the role
We are seeking an experienced Residential Care Officer to provide structured support, case coordination assistance, participant monitoring, community engagement and program delivery services for NDIS participants residing within Supported Independent Living (SIL) and residential care settings. The role focuses on promoting participant independence, wellbeing and community participation through person-centred support planning and service delivery.
Key responsibilities
- Assessing participant support needs and assisting in developing individual support and care plans.
- Implementing structured support programs designed to improve participants' independence, social inclusion and daily living capabilities.
- Monitoring participant progress against established support goals and maintaining detailed case records.
- Assisting participants to access community, health, welfare, housing, education and employment services.
- Liaising with families, guardians, support coordinators, therapists and external service providers to ensure coordinated service delivery.
- Supporting participants in developing life skills, communication skills and independent living capabilities.
- Conducting regular risk assessments and identifying emerging support needs.
- Preparing progress reports,
incident reports and documentation required under NDIS Practice Standards.
- Facilitating community participation programs and recreational activities that promote social engagement.
- Assisting with behaviour support strategies and implementing positive behaviour support plans where required.
About you
- Demonstrated understanding of client assessment, support planning, case management principles, and person-centred service delivery.
- Experience working within residential care, supported independent living (SIL), disability services, community services, mental health, youth work, or related welfare environments.
- Ability to assess client needs, monitor progress, and contribute to the development and implementation of individual support plans.
- Strong communication and interpersonal skills, with the ability to effectively engage with clients, families, guardians, support coordinators, allied health professionals, and external service providers.
- Experience in supporting individuals with complex needs, behavioural challenges, disabilities, or mental health conditions.
- Knowledge of community resources, welfare services, referral pathways, and support networks available to vulnerable individuals.
- Ability to identify risks, respond appropriately to incidents, and contribute to crisis intervention and conflict resolution strategies.
- Robust organisational, reporting, record-keeping, and documentation skills, including the preparation of case notes, progress reports, and incident reports.
- Sound understanding of NDIS Practice Standards, participant safeguarding requirements, privacy obligations, and duty of care responsibilities.
- Relevant qualification in Community Services, Social Work, Youth Work, Disability, Welfare, Human Services, Psychology, or a related discipline.
- Current NDIS Worker Screening Check, Working with Children Check (where applicable), First Aid Certificate, and valid Australian Driver's Licence.
Pay: $80,000.00 per year
