About the role

We are looking for a reliable and highly organised Case Manager / Service Coordinator to join our on-site Gold Coast team. Someone who is passionate about supporting others, takes ownership of their responsibilities and is committed to delivering consistent, high-quality care.

You will be a key point of contact for clients, families and support workers, combining individual case management with the practical coordination of daily services. From welcoming new clients and reviewing care arrangements to organising rosters and responding to changing needs, your work will help ensure clients feel heard and supported, and staff feel informed and prepared.

Working closely with management and administration, you will help maintain continuity of care, strengthen relationships and improve how we deliver our services. This role would suit someone who enjoys getting to know people, takes initiative and follows through with care and attention.

Key responsibilities

Client care and case management

- Build positive relationships with clients, families and representatives.
- Coordinate client onboarding, assess service needs and organise appropriate supports.
- Develop,



maintain and review care plans and risk assessments in collaboration with clients and relevant professionals, within your role and qualifications.
- Monitor client wellbeing, satisfaction and changing support needs.
- Coordinate services in line with client preferences, goals, service agreements and available funding.
- Liaise with support coordinators, health professionals, plan managers and other providers.

Service delivery and staff coordination

- Prepare and maintain rosters, arrange shift coverage and communicate changes promptly.
- Match support workers to clients’ needs, preferences and required skills.
- Ensure staff have access to current care plans, risk assessments and support instructions.
- Provide day-to-day guidance to support workers and escalate concerns appropriately.
- Review shift notes and follow up on service gaps or changes in client needs.

Quality, safeguarding and administration

- Record, follow up and escalate incidents, complaints and safeguarding concerns in line with organisational procedures.
- Maintain accurate, timely and confidential client records.
- Work with administration to monitor service utilisation and identify potential funding or billing issues.
- Support compliance with relevant NDIS and aged care requirements.
- Contribute to audits, service reviews and continuous improvement.

About you

Essential

- Experience in case management, service coordination or care coordination within disability, aged care or community services.
- A relevant qualification in Community Services, Disability, Aged Care, Social Work, Nursing or a related field, or equivalent relevant experience.




- Understanding of person-centred care, client choice, informed consent, dignity of risk and safeguarding.
- Working knowledge of relevant NDIS Practice Standards and aged care quality requirements.
- Experience coordinating care plans, client reviews and service arrangements.
- Strong organisational skills, with the ability to manage competing priorities and last-minute roster changes.
- Clear, respectful communication and a calm approach to resolving concerns.
- Confidence using client management systems, rostering software, Microsoft Office and email.
- Australian work rights and availability to work on-site during the stated hours.

Desirable

- Experience across both NDIS and aged care services.
- Experience supporting clients with complex needs.
- Familiarity with Brevity or similar care management software.
- Experience monitoring service budgets and utilisation.
- A current driver’s licence for any required client visits.

Pre-employment requirements

- A current NDIS Worker Screening Clearance.
- Additional screening relevant to the role and clients supported, including a Blue Card where applicable.
- Completion of the NDIS Worker Orientation Module, “Quality, Safety and You,” or willingness to complete it before commencement.
- Satisfactory professional reference checks.
