About the Role
My Help Services is seeking a qualified and compassionate Community Clinical Care Registered Nurse to provide clinical care and nursing oversight for older Australians and NDIS participants in their homes.
The successful candidate will conduct clinical assessments, develop care plans, provide nursing services, monitor client outcomes and support care workers to deliver safe and high-quality services. This role requires strong clinical judgement, effective communication and a commitment to person-centred care.
Key Responsibilities
- Conduct comprehensive nursing and clinical assessments.
- Develop, implement and regularly review individualised clinical care plans.
- Deliver nursing services in clients’ homes across Melbourne and regional Victoria.
- Monitor clients’ health, wellbeing, risks and changing care needs.
- Provide wound care, continence support, medication assistance and other nursing interventions within professional scope of practice.
- Support clients with chronic conditions, complex care and high-intensity support needs.
- Complete clinical documentation, progress notes, risk assessments and incident reports accurately and promptly.
- Identify and escalate clinical deterioration, safety concerns and changes in client condition.
- Liaise with clients, families, general practitioners, hospitals, allied health professionals and other healthcare providers.
- Provide clinical guidance, education and competency support to care workers.
- Participate in case conferences, clinical reviews and care planning meetings.
- Support medication management and medication-related risk monitoring.
- Assist with infection prevention and control practices.
- Respond appropriately to incidents, complaints and clinical emergencies.
- Maintain client privacy, dignity, choice, cultural safety and confidentiality.
- Ensure services comply with relevant legislation, professional standards, organisational policies, NDIS requirements and aged-care quality standards.
- Participate in clinical audits, quality improvement and continuous professional development.
Essential Qualifications and Experience
- Bachelor of Nursing or an equivalent recognised qualification.
- Current unrestricted registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- Demonstrated nursing experience in community care, aged care, disability support, home care or a related clinical setting.
- Sound clinical assessment, care planning and risk-management skills.
- Knowledge of person-centred, evidence-based and culturally responsive care.
- Strong written communication and clinical documentation skills.
- Ability to work independently and make appropriate clinical decisions.
- Strong interpersonal skills when working with clients, families and multidisciplinary professionals.
- Current National Police Check.
- Current NDIS Worker Screening Check or willingness to obtain one.
- Current First Aid and CPR certification.
- Valid Australian driver’s licence and access to a reliable, insured vehicle.
- Current Australian working rights.
Desirable Criteria
- Experience supporting Support at Home clients or NDIS participants.
- Experience with complex care, chronic conditions or high-intensity supports.
- Wound-management and continence-care experience.
- Medication-management knowledge.
- Experience conducting clinical competency assessments for support workers.
- Understanding of the NDIS Practice Standards, Aged Care Quality Standards and relevant Victorian requirements.
- Ability to communicate in a language other than English.
