Leaving hospital is one thing. Getting home safely and staying there is another — and that's exactly what this role makes possible.
As a TCP Coordinator with Kanda, you'll guide clients through the Transitional Care program — from initial assessment through to care plan delivery and ongoing coordination. You'll work closely with hospital teams, allied health professionals and funding bodies to make sure every client has what they need to recover at home, safely and with confidence.
It's clinical, it's coordinative, and the difference you make shows up in someone's ability to stay in the home they've built.
What you'll be doing
- Conducting comprehensive assessments and establishing or reviewing TCP care plans in line with program guidelines
- Liaising with hospital TCP coordinators, discharge planners, allied health professionals and funding bodies to coordinate total care
- Arranging services, monitoring client progress against health care goals and delegating clinical tasks in conjunction with the Senior Clinical Nurse
- Providing clear, accurate care plans for field staff and attending hospital TCP case management meetings
- Managing referrals, occupancy rates, invoicing and weekly reporting for the hospital TCP team
- Identifying and reporting risks, incidents and complaints, escalating unresolved concerns to management
You're the right person if...
- You hold current unrestricted AHPRA registration as a Registered Nurse
- You have previous experience in case management,
care coordination or a similar clinical role
- You have an understanding of aged care, My Aged Care portals, referral pathways and community supports
- You communicate with clarity and empathy — with clients, families, hospital teams and stakeholders
- You're highly organised, detail-focused and able to manage competing priorities without letting things slip
- You hold a current Drivers Licence, National Police Clearance and Working with Children Check, or are willing to obtain them
