The Client

Pride Disability Services Pty Ltd is a NDIS Registered provider. They provide individualised care and support to their participants with disability mainly in Melbourne.

As the demand for such disability care services grows in the region, they seek a passionate and qualified Community Support Worker to join their team.

The Role

This is for a full-time basis with an annual salary set at $75,000 to $85,000, depending on experience plus superannuation. The role will be based in Melbourne in VIC.

Responsibilities

- Assess clients' needs and develop individualised support programs.
- Interview clients to understand their challenges and strengths.
- Plan, deliver, and evaluate community-based programs, workshops, and activities.
- Monitor client progress and prepare progress reports.
- Refer clients to external agencies for specialised assistance when required.
- Assess community needs and identify available resources in areas such as health, housing, employment, and training.
- Build partnerships with community groups, welfare agencies, government bodies, and local businesses to promote awareness of services.
- Support families and provide education and care for children, young people, and adults in community-based programs.
- Assist young people to address social, emotional,



and financial challenges.
- Prepare reports, funding submissions, and documentation to support service delivery

Requirements In order to be shortlisted for this position, the right candidate needs to have:

- Minimum Certificate IV in Community Services or related field
- Minimum 12 months relevant work experience as a Community Worker or in a similar community services role
- Minimum 12 months relevant work experience of case management, report preparation, and submission processes including NDIS documentation
- Minimum 12 months demonstrated experience working with vulnerable individuals, people with disabilities, or culturally diverse communities
- Minimum 12 months of experience dealing with mental and emotional disturbances and developmental disability
- Minimum 12 months of experience working with government agencies, non-profits, and other service providers
- Have a current or be willing to obtain a First Aid (HLTAID003 or HLTAID004) and CPR (HLTAID001) certificate
- Current Working with Children Check
- Current AFP Police Check
- Current NDIS Worker Screening Clearance
- Minimum two qualified references from previous employers
- Excellent communication and interpersonal skills
